Health Services at Modbury was originally established and located within GP Plus Super Clinic 77 Smart Road Modbury SA. We are committed to providing ethical, compassionate and inclusive care in a safe and welcoming environment for all patients. Important Information for Our Patients
Immunitstions
Routine immunisations are available for both children and adults. An appointment is necessary with the nurse and your doctor so please inform reception staff.
Sickness Certificates
Sickness certificates will not be issued without a doctor’s consultation.
Test Results
Patients are required to make an appointment to obtain or discuss the results of any investigations such as blood tests and x-rays unless otherwise directed by your doctor.
Messages
Reception staff are happy to relay requested information to your doctor. Any patient requests will require an appointment in clinic or via telehealth consultation (subject to eligibility.) If your call is urgent you will need to explain your situation to the receptionist who will deal with your call appropriately. Our practice does not communicate with patients via email as emails may not be checked on a regular basis.
